So.  Let's start off with what I DO know.

1. This was from the Red Bird Wine Bar, in Missoula
2. It was delicious
3. Someone else ordered it (but I tasted it)
4. It's gorgeous

I considered not posting it because I didn't have all the information, but I need to whittle this stack of pictures down!
